Guest Pass Rules

REQUESTS MUST BE RECEIVED BY THE FITNESS FACILITY BY 3/31/07. Offer in stores begins 12/29/07 and ends 3/31/07. Must be 18 years old or older to be eligible. Limit one guest pass per person per club. Start and end date, plus manager’s signature must be indicated to activate guest pass. Two-week guest pass is valid for 14 consecutive days after date of redemption. Offer valid ONLY at participating fitness facilities as listed on the centrumguestpass.com website. Guest pass usage is limited to the facility where it was redeemed. Guest pass entitles bearer to standard facility privileges. Additional charges may accrue for additional services. Fitness center rules and some restrictions may apply. Valid photo ID required at check-in. Cannot be combined with any other offer. By accepting the guest pass, guest pass holder agrees to hold Wyeth Consumer Healthcare, Brooks, Duane Reade, Eckerd, the participating fitness facility, Promote It International harmless from, and each of their respective directors, officers, employees and assigns, against any claims and liability arising out of use of guest pass for any injuries, including injury resulting in death, however caused or sustained resulting from use of such facilities. Offer has no cash value.

Theft, diversion, reproduction, transfer, sale or purchase of this Offer Form, proofs-of-purchase or cash register receipts is prohibited and constitutes fraud. Void where taxed, restricted or prohibited. Proofs-of-Purchase submitted by clubs or organizations will not be honored or returned.
